donation to ASSET-Eight, we are able to further extend our educational
outreach.
You
may register for an online course at www.pbs.org/teacherline.
After you register for any PBS TeacherLine Course with fees of
$169 or higher,
please follow the steps outlined below to receive your rebate.
Steps
for receiving the $60.00 rebate:

Please
print the Arizona State University W-9 form. You must fill
out both pages of the form. The form must be signed and
either mailed or faxed back to ASSET. Click
here for the W-9 form.

may apply for the TeacherLine Course Rebate from day seven
of the course through the official end date of the course.
Your W-9 must be received by ASSET within this time frame.
Note:
if you receive a course refund from PBS Teacherline, you
are not eligible for the ASSET rebate.
